Chondromalacia patella, also called patellofemoral pain, is a common cause of chronic knee pain due to the softening of the cartilage under the kneecap (patella). This cartilage helps the patella to glide smoothly over the lateral end of the femur (thigh bone). In chondromalacia patella, poor patella alignment causes the the patella to rub roughly against it, creating friction and rough surface cartilage. The cause may be a slight existing misalignment of the patella, arthritis, overuse or injury. Symptoms include pain and swelling, often aggravated by activities such as running, climbing or descending stairs.
